They are all probably deductible. The details are in

Default Medical Expenses Incurred In Foreign Country

Would someone please care to sum up the tax deductibility of the
following as it relates to a taxpayer who travels to a foreign
country to be treated for or have surgery for a diagnosed illness.

Cost of foreign licensed doctors & nurses.
Cost of prescription drugs prescribed by foreign doctor and
administered in a foreign country.
Cost of hospital services & labs.
Cost of out patient facility.
Cost of rehab facility.
Travel expense to go and come back.
Lodging and meals.
