There is no requirment to file unless she would get a
refund. BTW, an extension was not required to get a refund.
She has three years from the due date of the return to file
and get a refund.

Filing could result in a refund of $36-40 from the Earned
Income Credit.

Single, age 57, no dependents, made about $500

I do not feel comfortable answering this post without
knowing what she made and whether she had any dependents,
whether she was married, and whether she was claimed as a
dependent on another return

She still could be entitled to a refund due to the earned
income credit.

I have a friend who lost her w-2 and called me on the 17th
asking what she should do. Based on what she told me, I
knew she wouldn't be required to file (didn't make enough),
but advised her to file an extension so she could file for a
refund. When she found the w-2, turns out there was no
federal withholding.

Does the IRS expect a return since she filed an extension,
or can she just not file anything?

If she does file, should she file a state return (KY) -
again she wouldn't meet filing requirements (and KY just
accepts the Fed extension).
