There is no place to put the income from a form 1099R on a
form 1040EZ. The least you can use is a form 1040A. Or you
may use a form 1040. But you may not use a form 1040EZ.

If you were issued a 1099R, this must be reported on a 1040
or 1040A. If you owe a penalty for early distribution (i.e.
you are younger than 59-1/2) then this must be reported on
form 1040. On either form, you add tax withheld to any other
tax withheld, e.g. from wages, and report that total amount
on the form.

There is no place on a 1040-EZ to reflect your 1099-R
distribution. Therefore, 1099-R distributions and taxes
withheld go on either a 1040 or 1040A.

The 1040EZ is designed for specific filers -- and that does
not include those receiving retirement income of any kind.

You can use Form 1040A, if you have only a 1099-R, and
therefore a simple tax situation.

Do you see a line on the 1040EZ form for entering your
retirement or IRA income (1099R)? Probably not, since the
form only accomodates wages, tips and some interest income.
Use form 1040.
