But you may not use a form 1040EZ. | If you were issued a 1099R, this must be reported on a 1040 or 1040A. If you owe a penalty for early distribution (i.e. you are younger than 59-1/2) then this must be reported on form 1040. Probably not, since the form only accomodates wages, tips and some interest income.
