For medical deductinons the percentage is 7.5%, not 7%.

Yes. If you're itemizing on Schedule A, you should include
health insurance premiums that you paid for your family.

Yes, medical insurance fees contribute to your (potentially)
deductible medical expenses.

Yes, provided your medical expenses exceed 7.5% of your AGI,
AND assuming that all your itemized deductions are more than
the standard deduction for your filing status.

This is all explained in the Schedule A instructions.
