You would only need to complete the 8606 to establish your
cost basis when you rollover a pension to an IRA. No 8606
is filed for a pension to pension rollover. This type of
rollover should be made as a direct rollover plan to plan.
As long as the new plan allows transfers of previously taxed
funds, the new plan should pick up the correct balances.

If a person has a 401k from a prior job that he wants to
roll over into the 401k for current job - and that prior
401k has a pre tax and after tax component - one, can you
commingle and two, how do you document that. I'm told you
might have to file a Form 8606 for the after tax component?
