You may acquire this form at http://www.irs.gov. Find on that website where you may download forms and instructions. Get that form and depending on what State you live in you may be able to amend that State's return also (if that State has an individual income tax). In your situation, you would note the change in amount of deductions, explain it on page two by reference to the appropriate line number, and also attach a revised Schedule A with the proper figures.
