Just ill ou the 1040X with an explanation. NYS, use IT
201X, you must use the printed form for the year unlike the
1040X.

Do I have to send in the entire tax return, or just the
forms that have changes? (federal and NYS) Is it okay to
photocopy the old tax returns and make the changes with
whiteout? If not, how do I get old forms? On the internet
somewhere? To save a little postage, can I send in 3 years
in one envelope, or must I do everything separately? Do I
include a cover letter explaining what has changed? Any
other tips?

In doing this years returns I found an investment advisor
had never sent me documentation on his fees, so I didn't
deduct them in 2001, 2002, and 2003. It only affects one
line on Schedule A, and the consequential lines on 1040 (and
their NYS equivalents) It is not a huge amount of money,
but probably worthwhile.
