You only have to include the forms that changed since the
original return. If Schedule D didn't change you technically
don't need to file it, however, because certain entries from
Schedule K-1 usually pass through to Schedule D in my
opinion it would be a good idea to include it.

And that's because the software can't tell what's what. When
I amend a return, I selectively print just the forms,
schedules required. I venture to say most software will
let you do that, using usual Windows conventions, such as
right clicking the mouse for a pop up menu and then select
"print this form", and then the number of copies to print.
Try it.

Just the modified forms -- at least according to the
instructions and my experience. Of course, if the (normal
25%) bracket calculations are involved, then Schedule D DOES
need to be recalculated....

Just the 1040X and any affected forms. Don't waste any
postage.

Just the 1040X is needed for IRS, if any formed changed, you
might want to keep for future reference.

I just include a new schedule, a new form and a changed
schedule or form only if the change can not easily be
explained on page 2 of the 1040X. E.g., I don't send a new
Schedule B just because some interest or dividends were
overlooked on the first pass. I just note the change on
page 2 of the 1040X where it asks for the explanation. I do
include a changed Schedule D as it is easier to attach the
changed schedule rather than provide all the data in the
explanation.
