The only reporting requirement for a QTP is when you have a
taxable distribution (Line 21 of the 1040). I would just
attach a statement that states the QTP distribution was
rolled over to either an account for the same beneficiary or
another member of the family. I am not aware of any
requirement to report a contribution to a CESA.

Default 1099-Q

I have received 1099-Q, the amount was fully rolled over to
another mutual fund company which makes it non-taxable. Do I
have to report the transaction somewhere and if so where? My
daughter doesn't need to file her own tax return.

And one more question, do I have to report contributions for
Coverdell ESA and if so, how do I do that? We used to report
it on our tax return when the Coverdell ESA was still
Education IRA but now I cannot find any reliable information
on the subject.
