IRS used to have that form for pension payments, but they are now reported on a 1099R. Not sure about what you mean about "not being a team player", certainly not a tax term. Is it possible then, that his pension or a portion thereof has been awarded by a court in what's called a QDRO? If so, that portion will be taxable to her.
