You are correct. You have to add the ordinary gain to Line
7 wages. Just enter the word "ESPP" on the line.

Default Gift of ESPP stock to charity

I donated ESPP stock to a charity via a transfer of ESPP
stock from my name into the charity's name. The transaction
is a qualified disposition. Parameters are as follows: FMV
of stock at grant date = 15.00, FMV of stock at purchase
date = 10.00, purchase price of stock = 8.50 (15% discount
off of price at purchase date b/c less than 15% off of price
at grant date), FMV of stock at gift date = 25.00. I
expected to see 2.25 (lesser of 25.00 minus 8.50, or 15.00
minus 12.75) in my W-2 from employer. But it wasn't there.
I assume my employer didn't know the transfer occurred, or
maybe my employer didn't know the FMV of the stock at the
date that the gift was completed. Nevertheless, I believe I
am supposed to include the 2.25 somewhere in my 1040 as
"ordinary income." Anybody know where I should put it ? (In
addtion, I will take 25.00 as a noncash donation of stock to
the charity.)
