Read and follow the form instructions.

Generally, you only need to attach schedules, statements,
etc. if they are different from those originally filed. If
the change for which you need to file the amendment affects
a schedule, generally you should attach two copies of that
schedule, one clearly marked "AS ORIGINALLY FILED" and
another marked "AS AMENDED."

If the change results from a corrected W-2, of course you
should attach the corrected one.

Default Amended state return

I have to file an amended NYS return. Do I have to file all
the forms and background W2's, etc. again or just the
amended IT-201X.
