Thank you both. I don't think the IRS will have processed
our ITIN application before April 15 (that falls beneath
their minimum processing time). I think the only option I
can see here is to contact the AZ department of revenue to
see what they'd want. Unfortunately, I can't find a phone
number for them. How did you go about contacting your
state's tax office? Does anyone know of a way to contact
the AZ state tax office with an inquiry like this?

My state tax offices (PA and CA) asked me to send in a
'copy' of the W-7 form with the state taxes. They probably
will be able to pull the ITIN number by the time they get to
processing the returns.
