You file a Form 1040 with Schedule D once the court has
notified you that you'll get no money. If this was a prior
year, you'll need Form 1040-X, amended return.

Default capital loss/bad debt/ammending a return

I was told I may deduct a bad debt incurred because of
someone's bankrupsy. How do I go ahead and ammend this
return? I know I need tax form "D".

Thanks for your help.
