Not this year: The mailing deadline was February 2, 2004,
due to the weekend rule.

Most probably the rollover was direct from trustee to new
IRA trustee, in which case no 1099-R, hence no reporting
requirement.

Yes. But you will quite likely receive a 1099-R soon,
anyway. With the official required mailing deadline of
January 31, many of those straggle in over the next week or
ten days.

Wife rolled over Profit Sharing account from previous
employer to her Rollover IRA, however no 1099-R was issued.
Should I still include this on line 16 as if we had a
1099-R?
