You need to withdraw the excess $275 by the due date of the
tax return. Make sure the administrator of the 401k
understands that you are withdrawing an excess contribution.
You have to include the $275 in income on your 2003 tax
return.

See Publication 525. You'll add the $275 to your W-2 wages.

Default Overcontribution to 401k question

Due to changing jobs 4 times last year, I actually
overcontributed to my 401k by $275.00 beyond the 12K
permitted. My company's payroll isn't interested in helping
- how can I remedy this on my tax returns and ensure I pay
the extra taxes when my W2 shows I overcontributed?
