Put it on Schedule B, or if you aren't required to file
Schedule B, on the line for interest on whichever version of
a 1040 you file. No special statement required.

You don't attach 1099's to returns if they don't show
withheld tax. Just report the interest income on the
appropriate line of your return.

Default Self-reporting interest income - how to?

In a situation where a non-resident alien becomes married to
a US resident, and they decide to file a joint tax return
(requesting that a non-resident be treated as resident) how
can they report interest income from bank accounts?

Non-resident had a bank account in US registered as such
(not sure what form it is, W8 or some such) and no taxes
were withheld on interest income in previous year. No 1099
was issued either and a bank won't issue one now.

What is the best way to report the interest income from such
account without 1099? I considered just writing up a
statement with a total amount of income, account number,
person's SSN etc and attaching it to tax return. Would this
trigger any problems?
