Default requesting Medical Reimbursement Plan document

Would anyone out there post (or email me) examples of
Medical/Dental Reimbursement Plan documents appropriate to
an C Corporation with one owner.

I'd like to upgrade mine, which is very old.

The corporation is in Illinois, if that matters (I suspect
that it does not).
