Entity name, address, tax ID number, the tax year, and the
form/schedule and line number the schedule relates to.

IE:

Joe Company, Inc
1234 Nowhere Road
Somewhere, State Zip
12-3456789
Tax Year: 2003
Form 1065, Schedule, Line

Default 1065/Generic schedule question

I am trying to fill out a 1065 plus the K-1 schedules this
year. I understand them OK, and they seems fairly straight
forward. However, occasionally when it wants something,
i.e.:

K-1 line 7 or K-1 line 11

It says to attach schedule. So, do I just make one up, or
does the IRS have some type of generic one that would work.
I guess what I am really asking is, I know the schedule
would list the items, but what other information is needed
on the schedule? Name? ID number?
