They had better do that.

Exception is if you're a bona fide sub contractor, and not a
"common law" employee. In short, if you work on their
premises and under their daily control for same amount of
hours per day, you ARE an employee.

In general, yes. In a few special cases, perhaps not.

Unless you fall into one of the religious exemptions a non
profit employeer is required to withhold SS and Medicare
etc.

What does your w-2 say?

They should, if you are an employee.

Yes, unless the non-profit is a church and you are
clergy employed by said church.
