You will receive 1099-R's for these payments. You report
the income as retirement plan income (line 16 of the 1040)
and the withholding the same way you would report wage
withholding.

Default Benefit Payment Tax Questions

I had some retirement plans closed out for me from a couple
of companies I used to work for. I elected to have the
distributions made directly to me as I was not working this
year.

I am not retired (age 33) and am not sure how to report
these payments made to me for my taxes this year. Also, I
am unsure how to report the withholdings that were made for
these payments (there was both federal and state
withholdings automatically made).

Should I be receiving any documents related to the payments,
a 1099 perhaps?

Any help would be appreciated,
