Another homework problem, eh? Pub 15 should be able to
answer your queation.

Sure, if you have enough income other than from your employer.

If a salaried full time employee with one employer and one
W2, specifies 0 exemptions and married withhold at single
rate at start of a year, is there any circumstance where the
amount of tax withheld will not be sufficient to cover the
tax bill.

Does it make a difference if the income is $50k, $100k,
$200k, $300k, $400k? Does it make a difference if most of
the income is a bonus payment?

Assume all withholding of income, including bonuses, are set
by W4 not a flat rate. Assume itemized deductions are
similar in size to standard deductions, and AMT will not
apply.
