Yeah, I recently went through the same thought process. I decided it's OK to
leave these as "transfers out of budget" on the basis that my budget shows
what I'm spending, rather than what I'm moving around.

You should exclude your 401(k) accounts from your budget accounts. If you
have your 401(k) contribution set up as part of your paycheck, then the
transfer out of budget accounts in the budget should show this transfer as
expected.

Well as far as the last part goes, add it as a line item to your
paycheck in Bills & Deposits. This assumes that your paycheck comes
regularly and that you've got one created in Bills & Deposits.
If you don't have one already create one. If you're paid on an
irregular paycheck (e.g. paid on commission) then estimate your
pay on a monthly basis.

If you have a matching component from your employer, you should do

Savings in the budget = Income + transfer in - expenses - transfer out -
debt service.

You may be able to setup the 401k cash account as in budget--but I wouldn't
do it.

Default Budget: Transfers Out of Budget Accounts (401k)

I have an automatic deduction from my payroll to my 401K, and am tracking
Gross Amount Payroll salary postings. In my budget however, the 401K shows
as a tranfer out to another account and I cannot figure out how to actually
account for this through the savings portion of the budget or match it as a
regular transfer out that is expected.
