Default 401K questions

I am a foreigner working in the USA for 5 years now. I have some money
in my
401K account. I am planning to return to my country permanently this
year.
My 401K is with Fidelity.

I have some questions about this

What happens to my 401K money when I leave the USA - i.e. do I have an
option
of withdrawing it (After paying a penalty) & taking it with me. Even if
this is
allowed, assuming I have no need for the money now, is it better to
leave it in
a 401K account/IRA etc till I am 59 & then take it out without penalty.

Are these accounts insured? i.e. if 5 years down the line, Fidelity
goes bust
(bankrupt) what happens to my money?

If 5 years down the line, if I have some financial problems, will I
then be
able to withdraw it (with/without penalty) - how difficult will it be
to
conduct these operations from a different country?

Does anyone have any experience with this?
